If you do not complete and submit this form (or complete and return the card received by mail) within 30 days, you may have to affirm or confirm your address the next time you vote in a District of Columbia election, and if you do not vote in one of the next two general elections, your voter registration will be canceled and you will need to re-register to vote in future District of Columbia elections.

Please keep in mind that if you moved outside of the District of Columbia, you will need to register at your local election office to vote where you live.
